Kula for Karma is a not-for-profit corporation organized under the laws of New Jersey to provide yoga instruction to individuals in challenging life circumstances.

I verify that the above information is true to the best of my knowledge. I agree that my name and likeness may be used by Kula for Karma in its promotional materials.

*[INSTRUCTOR] hereby represents and warrants that he/she is a certified yoga instructor through a program approved by the Yoga Alliance.

[INSTRUCTOR] further represents and warrants that he/she will maintain and provide evidence of general liability insurance coverage through a reputable insurer while he/she provides yoga instruction for Kula for Karma.

[INSTRUCTOR] shall make good-faith efforts to have Kula for Karma named as an additional named insured on such policy.

[INSTRUCTOR] acknowledges that he/she has reviewed the “Policies and Procedures ” document and agrees to abide by the expectations set forth in the document.

From time to time, Kula for Karma may take a photograph and publish it for the purpose relating to the organization. [INSTRUCTOR] consent’s to his/her picture being used for such purposes and understands that his/her name may be used.

Additionally, [INSTRUCTOR] consents to not photograph any Kula for Karma clients.

[INSTRUCTOR] releases and covenants not to sue Kula for Karma, and agrees to indemnify, defend and hold harmless Kula for Karma, its directors and officers, from and against any all costs (including reasonable attorneys’ fees), liabilities and claims arising from any injury to persons or damage to property to the extent caused by INSTRUCTOR’S negligent acts or omissions or willful misconduct while performing services for Kula for Karma.
